Excel Viewer 2003
Get the free Excel viewer to open Excel spreadsheets, even if you don't have Excel.
Runs on Microsoft Windows XP, Windows 2000, and Windows Server 2003.
Installation instructions: Click this link, then click the Download button on the Microsoft site. In the box that opens, click Save (not Run). Save the file somewhere on your computer that you can find again (like your Desktop or My Downloads). When the download is complete, navigate to the file you just saved (such as on your Desktop), and double-click it. An installation program will start and will install Excel Viewer on your computer. Accept the defaults. Next, click the Excel file that you want to view (such as a spreadsheet) and click Save (not Open). Save the file to your Desktop. Then go to the Start menu and open Excel Viewer. From the Excel Viewer, open the Excel file you just saved to your Desktop. |

Excel. Runs on Microsoft Windows 98 or earlier.
See the installation instructions above for Excel Viewer 2003. The only difference is that you will not have the first step of going to the Microsoft site. Your instructions being with the box that opens.

Birth certificate authorization form
Either this form or the original birth certificate or a certified copy of the birth certificate from the governement is due on March 4 from players. This form is used to allow Karen to obtain a certified copy of your player's birth certificate on your behalf. Please email or fax this form to Karen by March 4. Please note that if you use this form your license must come in clearly on the copy or the county clerk will not accept it. You must also send $23, payable to Karen Farrell, to cover the fees. Instructions are on the form. |

Health History and Exam Form
Due from players and coaches. Players must have this form signed by a medical practioner, but this signature is not required for coaches. Wildcat Pushup Chart
Chart and instructions for weekly pushup program. Players receive cards for every 10 pushups they average per week. Players without a record of pushups at Friday practice must do an extra 30 pushups at practice. Charts can be recorded on any type of paper, but please initial each day, put totals at the end of the week, and average the week. Start on Friday, and end on Thursday (or just use this chart). There is a 210-pushup per week minimum to earn cards (average of 30 per day). |
