Cooperstown Required Information - Most Due by March 4

Congratulations on being selected to go to Cooperstown Dreams Park with the Texas Wildcats! Please complete the information below because we have to give all this information to Cooperstown. Sorry!

Cooperstown Required Forms

In addition to the below online form, you must complete the following paper forms and mail them to the address below. Please have these back to me no later than March 15. If you are going to be out of town for spring break, please get these to me right away, before you leave!

For Players

waiver & release
(requires both your and your player's signatures)
health history & exam form
(requires medical practictioner's signature, copy of insurance card, front and back, and shot record)
consent form
(please list all three coaches for consent: Patrick Farrell, Gregory Ramsey, and Rafail Hernandez)

Note: The next form requires a height and weight for your player, so when you go to the doctor, you might ask him or her to get your player's height and weight. The health history form does not ask for it.

Original or Certified Birth Certificate (Not a Copy)

We must also have an original or a certified copy (from the tax office) of your player's birth certificate (not a photocopy or hospital birth certificate).

Luckily, one of our golf course employees also works at the County Clerk's office, and she told us how we can accomplish this easily. All you have to do is complete this release form, which states that you allow Karen to obtain the birth certificate, tape your license to the form, and fax it to 866-229-0121 (or scan it and email it to karen@texaswildcats.org).

The photo on your license much be clear enough for the county clerk to see your face.

Then, because they only take cash, you need to mail a check for $23 payable to

Please have these to me no later than Wednesday, March 4—the sooner, the better!

If you prefer, you can mail an existing certified copy of your player's birth certificate to the address above. (And don't send $23.) All birth certificates will be returned after the tournament.

If you answered anything but Yes above, please give more detail about your method of travel, including airline, flight number, dates, airport flying into, arrival time, and number of persons flying. Please give information about split parties (if some are flying with group and others driving or flying on their own). If you plan to rent a car, please give the rental car agency. I don't know why they ask for all this, but they do... (See the airline information help below if you plan to fly on your own.)

If you don't plan to travel on June 5 & June 12, please type the dates of travel. Otherwise, leave blank. Note that players must be checked in by 11 AM on June 6! Also, group travel requires everyone to fly on the same date.

When do you plan to check your player in to Cooperstown Dreams Park? Cooperstown Dreams Park highly recommends taking advantage of Friday check in. Further, the houses in which we are staying are regulated by the New York Department of Health regarding number of occupants, and we are filling those houses just with fans. If your players stay with you on Friday, you will have to stay somewhere else with them—unless someone in your house is arriving on Saturday.

Please note that after everyone has submitted this information, there will be another form to complete with roster information. I'm sorry that I can't include those questions here, but their website won't let me access that form until I give them this information. Therefore, I don't know what questions to ask you yet. Sorry about that.

Please also note that one thing the next form will ask for is player (and coach) ring size, so to get a jump on that, please take your player (or coach) to the jeweler and have his finger sized. The smallest size ring they give is 7. I have a ring sizer if you'd rather meet in Alvin and size his finger using that sizer.
